Creating a Plan for Your Future involves many things that need to work together in order to have a meaningful life:

As family members, we wait to get started on this until it is too late. By taking some simple steps today and using the some of the Charting the Life Course tools, you can get started on documenting what you want life to look like for yourself or your son/daughter with a disability.

These tools are not “worksheets,” but they are meant to encourage conversations, think differently, raise expectations, integrate all types of support, and provide life experiences to help any person get to their vision of a good life. These tools can be used by anyone, whether they have a disability or not.

Roles adjust as the individual members of the family system change and age. The entire family, individually and as a whole, needs support to ensure they all are able to successfully live their good life.

Life domains are the different aspects and experiences of life that we all consider as we age and grow. These include:

Community Living
• Safety & Security
• Healthy Living
• Social & Spirituality
Advocacy and Engagement
